 Insurance

Insurance Plans Accepted:   Richard R. Rosenthal, M.D., LTD. participates with Medicare and most major insurance plans.  Please call our office for more information about which insurance plans we participate with.  You should also check with your insurance company for the most up-to-date information on preferred providers.  You are responsible for charges that are not covered by your insurance at the time of your appointment.

Payment:
  Any co-payment required by your insurance company is due at the time of your appointment.   If necessary, the insurance and billing coordinator will assist patients to set up a payment plan.

Referrals:  Treating the symptoms of allergy and asthma is a medical specialty and many insurance companies require an authorization, in the form of a referral, from your primary care physician in order to see a specialist.  If your insurance company requires a referral please obtain one for allergy/asthma symptoms, testing, treatment and at least three office visits.

 Patient Forms

To speed your check in,  new patients are encouraged to print the new patient registration form, new patient questionnaire and office policy form and bring the completed forms with them to their first visit.  Forms may be downloaded from the Forms page.

 Prescription Refill Information

Misplaced or Lost Prescriptions

An administrative fee of $15.00 will be charged for the reissue of “misplaced/lost prescriptions” that were given to you at the time of your office visit. Please keep prescriptions in a safe place so that you can locate them.  Many patients, although given a prescription, call to have our office contact the pharmacy for them.  It is the patient’s responsibility to take or mail their prescriptions to their pharmacy.  If you cannot locate your prescription, please call the Patient Information Line at 703- 541-3326 with your request and allow two business days for our clinical staff to reprocess. There will be an administrative fee ($15.00) to reissue the prescription or to contact your pharmacy.

 

Mail Order Prescriptions

We are no longer able to offer the service of calling or faxing prescriptions to “mail order pharmacies”.  Please request this type of prescription (90 days) at the time of your office visit with the doctor so that you may send your prescription to your mail order pharmacy. If you did not request a “mail order” prescription or have lost/misplaced the prescription that was handed to you at your last office visit, there will be an administrative fee of $15.00 to reissue the prescription.  Please call the Patient Information Line at 703- 541-3326 with your request and allow two business days to process your request.

 

Medication Trial

Many patients are placed on a medication “trial” at the time of their office visit.  When ready to request the medication that works best, please call the Patient Information Line at  703- 541-3326 with your request.  Allow two business days to process your prescription. There are no administrative fees for this service.

 

Reston Office
If you wish to pick up a prescription in the Reston office, please leave this information on the Patient Information Line.   Please call the office to verify that your prescription is ready for you in Reston.  Prescriptions may take several days to process.

The information line is available 24/7 and is where all pharmacy requests should be left.  Please allow two or more days to process your request.  Additionally, office hours for allergy injections are also posted there, as well as emergency or inclement weather closings.

 

All prescription requests need to be called into the Patient Information Line.  We are unable to process walk in requests during injection hours.
